Membership fees and Payment method: Single Contestant $75.00Family Membership $125.00 (must live in same household... not cousins in different household)

1) Mail payment>>You must include with payment LEGIBLE name of all contestants or include the invoice sent. You may also include the original of the Contestant form and Waiver, notarized.*$45 charge for all returned checks

2) Pay online: By choosing this method below, you are accepting the convenience fee of $5.00, and you will receive an invoice at the above e-mail address****Once fee is assessed it will not be reversed, even if you change your payment method.

*A)$75 for individual or $125 for family I will pay by mail.                                                                                    B) (75+5 con. fee) INDIV MEMB Please send invoice I will pay online.                                                            C) (125+5 con. fee) FAMILY MEMB Please send invoice I will pay online - list the other family members under "other" but you still need to fill out their membership forms.                                                                D) I previously requested a family invoice.

**Either payment option you will receive a invoice. If selected to pay online for a fee you will be able to pay from it. If you selected to mail in check print off the invoice and send with payment.
